360 Degrees of Perfection's 5th Annual - Adopt a Family for Christmas NOMINATION FORM

360 Degrees of Perfection will be sponsoring One Family this Christmas. We know MANY MANY families are struggling this year, more so than other years, with Covid-19 hitting some families, very hard, and we wish we could help more, however we would like to randomly select one family, as we do every year, to sponsor for Christmas. Our staff will go shopping, wrap the presents, and deliver them to the family. 

PLEASE ONLY SUBMIT ONE FAMILY PER PERSON

WINNER WILL BE RANDOMLY SELECTED ON Tuesday, December  15th, 2020 and we will reach out via email to let the nominating person know, that the family they nomination for our Annual Adopt A Family for Christmas, has been chosen.

We will PUBLISH the winning families information, should they allow us to share their story, after the first of the year.
